  This document from AOTA enables the program to maintain current data about all clinical sites.  It is also helpful in gathering data required for ACOTE accreditation.  We request that each program update this form on an annual basis.  It can be saved electronically and emailed to mailto:mariann@scc.losrios.edu .
• Professional Behavior Evaluation
  For Level I Fieldwork Edcuators. Please complete the following evaluation for each Level I student. After reviewing the evaluation with the student, the student is responsible for returning this document to the Academic Fieldwork Coordinator.
