Thank you for considering membership in Phi Theta Kappa!

You are eligible for membership if you are:
- Currently enrolled at SCC in at least .5 unit.
- Have completed at least 12 units of course work at SCC in classes applicable towards an AA, AS, or BA degree.
- Have at least a 3.5 grade point average.

To receive an application:
Pick up an application form in the SCC Foundation Office, RN222 or in the wall file on the 2nd floor of Rodda South (near the bridge between Rodda South and Rodda North).

Read the following instructions carefully before you apply:
1. Fill out the first six lines of the form; social security number is optional.
Please print all information clearly, and be sure to sign the application.
2. Attach a copy of your grade report that lists classes you have taken at SCC
and cumulative grades received in those classes. You may obtain your
transcripts on line. You don't have to submit an official transcript.
3. Attach a receipt from the business office for $55 OR submit a copy of your
fee waiver letter or receipt from City College's Financial Aid Office.
Students on financial aid will receive a scholarship funded by the SCC Foundation.

To pay your fee, do the following:
If you do not receive financial aid, please pay the $55 fee at the Business Office on the first floor of Rodda North by Monday October 31st, 2011 at 5:00 p.m. Please turn in your application on or before this date to become a member this semester. Refer to the account number for Beta Eta Psi, the Phi Theta Kappa chapter at Sacramento City College, when paying fees. That account number is BANSC 5890 81 SC.VI.LLIT 64900 2009 407A. Again, use this account number to ensure proper credit. Be sure to attach your receipt to your application.

You must submit applications to the Foundation Office, RN222 by Monday, October 31st at 5:00 p.m.
