New & Reassigned Faculty Staff Form - The purpose of this form is to acquire the information necessary to provide computer access to SCC staff and faculty. Use this form when you have a new faculty or staff member working in your department or division. Click here to access form.
Employee Leaving Department Form - The purpose of this form is to acquire the information necessary to cancel access to IT resources for faculty/staff that will no longer be working in your department. Use this form when an employee is leaving. This includes retirements, resignations and transfers. Click here to access form.
