PTA Program Application Form

Applications for Fall 2020 are open to first-time applicants from August 24, 2019 until April 15, 2020. The application for returning applicants who have an Alternate Candidate Number from 2019 will be open from March 15, 2020 – April 15, 2020.

SCC PTA Program Application

  • Please carefully read all instructions and complete all required steps. Applications are accepted until April 15th for each year's Fall Semester start. Applications received after April 15th will be processed for the following year. First-time applicants and those applicants without an alternate candidate number from 2018 must submit a completed application to be considered eligible for enrollment in the PTA program.

    A Los Rios ID is required to apply. If you do not have a Los Rios ID, please apply to the college

    As part of the application process, applicants should submit official transcripts to show evidence of completion of program prerequisites. Official transcripts do not need to be submitted for any prerequisite courses or degrees completed through the Los Rios Community College District (American River College, Cosumnes River College, Folsom Lake College, Sacramento City College).

    Transcripts can be submitted at any time prior to applying. Please submit transcripts to:

    Upon submission of an application, confirmation will be sent to the email address provided. It is the responsibility of the applicant to ensure that applications are submitted accurately and completely. Program administrators are unable to confirm correct submission of an application.

PTA Program Prerequisite Courses

  • Complete the fields below for all prerequisite courses. Prerequisite courses must be completed with a grade of 'C' or higher in order to be considered eligible for enrollment. Prerequisite GPA must be at least 3.0. Anatomy & Physiology must have been completed within the past 10 years. The final semester of Anatomy/Physiology may be in progress at the time of application.

    Applicants must submit official transcripts to the Office of Admissions & Records as evidence of completion of program prerequisite courses taken outside of the Los Rios Community College District. Please visit the PTA Program website for more instructions on how to submit official transcripts.

    Applicants who have taken prerequisite courses at an institution outside of the Los Rios Community College District that are not on the Equivalency Grid must submit a course substitution form with their official transcripts. Please visit the PTA Program website for information on how to submit a course substitution.
  • Anatomy/Physiology (BIO 430 or equivalent)

  • Anatomy/Physiology (BIO 431 or equivalent)

  • Anatomy/Physiology (BIO 430/431 or equivalent) Complete this section ONLY if quarter-based A & P courses have been taken. Include information for 3rd course, only if applicable.

  • Introduction to PTA (PTA 100)

  • College Composition (ENGWR 300, ENGWR 488, ESLW 340, or Equivalent)

  • Information on ethnic background and gender is requested for statistical purposes only. Providing gender and ethnicity information is voluntary and does not affect program eligibility.

  • Prior to submitting your application ensure that you have accurately and completely filled out all required fields.
